Please provide deatiled information as following:
1) UltraISO version (for example, 8.6.1.1985, choose 'Help'->'About' from main menu)

I have the same problem
1) 8.6.1.1985
2) DVD+R
3) Philips DVDR 1660P1

I am using Windows ME with FAT32 and I don't know if the full size of the DVD is bigger or smaller than 4GB (very close). I made a backup of the DVD as .isz in 2 GB blocks and tried to burn a DVD from it now.
This happened both with the older driver version (1.4) and after I updated the Philips driver (1.6).

Same problem again. This time I found a way to fix it: by not using isz compressed form.
